How patient support works

Integrated features

Our unique concept is founded on personal health guidance in combination with the patient having access to their own web-based health journal for their own health activities and follow-ups.  The communications platform from Health Solutions is utilised for communicating effectively with the patient in between the telephone contacts.

The various patient support programs are tailored by combining telephone calls to the patient, messages via e-mail and SMS, and features specific to the diagnosis in the Health journal. This integrated feature set makes it possible to provide individually tailored patient support based on the specific needs and situation for each patient, such as diagnosis and length of treatment.

Health support center

A patient participating in one of our patient support programs has their own personal guide at the Health Solutions health support center. The personal guide follows the patient’s progress and their health activities over a long period of time. Guidance is primarily given through motivating talks by phone.

The support center’s nurses and health guides help the patient in both adherence to their pharmaceutical treatment as well as changes in lifestyle factors such as stopping smoking, changing food and exercise routines, motivation and stress management.

The continuity generated in the contact between the patient and their guide makes tailored guidance possible and fosters secure and motivated patients.

Health journal

The patients get access to their own health journal online. The journal is used as a support tool and as documentation of health activities carried out by the patient. Here the patient can enter their own values and details and follow progress. The journal is shared between the patient and the guide in question.

Communication platform

Health Solutions’ communication platform deals with the messages sent to the patient via e-mail and SMS. The platform is integrated with the support center and health journal so that communications are synchronized with motivating calls and events which the patient reports in their journal. By utilising the possibility to automate messages in the patient support tool for various events, communication with the patient is made more effective.
